Thank you for your interest in our drawing for a FREE set of 1,000 custom designed Munici-cards with a value of $80.00!

We will award some lucky individual or organization one set of 1,000 custom designed Munici-cards! These are the same high quality, fully customized Munici-cards which we have been producing for our customers.

In order to enter our drawing, you must read and agree to these rules:

1. Only one free set will be given away per organization.
2. The entry form MUST BE COMPLETED IN ITS ENTIRETY. Entry forms received without the required, valid information will be INELIGIBLE for the drawing.
3. The organization will be responsible for supplying a photograph to be used on their card. In addition, the organization will supply any logo or patch and any personal information it wishes to include on their card. Required information must be received within twelve months of notification of winning or winner will forfeit their FREE set of Munici-cards.
4. The person registering their organization is authorized to act on behalf of their organization and is authorized to enter their organization in this drawing.
5. The winning organization will be notified via US Mail and email to the address supplied on the registration form and the winner will also be posted on the winners page. In addition, you agree to allow the publication of your organization's name (if a winner) in printed material and/or on our website.
6. Munici-card reserves the right to end this program without prior notice and to modify these rules as necessary.
7. I understand and agree that I must claim my free Munici-cards within one year or I will forfeit the right to claim the free set of cards I have won and acknowledge that I have read and agree to the above registration rules.
